To assess the association of reactivation of varicella zoster virus (VZV) with systemic lupus erythematosus (SLE) or lupus nephritis (LN) and the effect of immunosuppressive agents on the course of infection. A review of medical records of 42 patients with SLE followed in our outpatient clinic was conducted. Five patients had reactivation of VZV, of whom 4 (80%) had a past history of SLE, 4 (80%) had LN, and 2 (40%) had both. In addition to rash, fever and pruritus occurred in all patients. The courses of VZV infection were relatively mild, except for 1 patient who had diffuse interstitial pneumonitis and neuropathy. No patients died of the infection. SLE was active in 3 of the 5 patients (60%) and renal disease was active in all 5 patients (100%) in whom lupus nephritis was documented.
